Below are links to commonly used forms that may be downloaded  or completed online.  Simply click on the text link of the clipboard image.  Completed forms may be returned to the Shepherdsville Police Department's front desk between the hours of 8:00 A.M. and 8:00 P.M. Monday through Friday.

House Watch Form
For directed patrols of your home while you are away.

Records Request
Print out the Records Request form and bring it with you to our office.

Civilian Traffic Collision Report
In the event of a traffic accident not requiring investigation by a police officer, this form may be completed and returned to the Kentucky State Police.

 

Email Alert Form
This form may be returned to the SPD front desk or emailed to mfrank@sheppolice.com.
